Frozen Fusion: Company Optimization Report
Jacksonville, FL. Frozen Fusion Cafe, 10503 San Jose Blvd. Prepared by Sandra Restored It. Graded October 10-11, 2026, from public records and public pages only (no owner input yet). Every risk below was double-checked by a second reviewer.
Overall: no overall letter yet, on what the public can see.
We checked 493 items. 453 apply to a dessert, boba and food shop with a beer-and-wine license. We could grade 48 of them from public sources (11%). Another 13 are estimates that need your confirmation, so they are shown but left out of every grade. 392 need your records or your answer to a question, 0 are still waiting on a check from outside, and 40 don't apply or are counted on the listings page. A letter needs at least 25% of the items that apply to be graded, and 11% could be graded from outside, so there is no overall letter yet. Each area below shows its own letter where it has enough graded items. The Social media row (M4) and the What's trending area are left out of the grades for now. Send us your page insights for both.
ESTIMATE means: we are not sure; please confirm. Grades use the 4.3-point scale (A+ 4.3, A 4.0, A- 3.7, B+ 3.3, B 3.0, B- 2.7, C+ 2.3, C 2.0, C- 1.7, D 1.0, F 0).
Every item we checked is listed in its area with what we saw, the date, why it matters and the fix (493 items in 28 areas). Items that only the owner can answer (books, payroll, staff) are collected as questions at the end.
Fix right now (6 risks)
Times and costs below are our estimates.
1. Your web address expires November 4, 2026, and renewal shows as locked. The registry record (read October 10, 2026) shows the address ends November 4, 2026, its first renewal, with a "client renew prohibited" lock that can block renewal. If it lapses, the website, online ordering and any email on the name stop. Ask GoDaddy to lift the renewal lock this week, then renew for several years and turn on auto-renew. Question for you: Who holds the GoDaddy account for frozenfusionus.com, is auto-renew on with a valid card, and why does the record show "client renew prohibited"?
2. Workers under 18 at a business with a beer-and-wine license. A business with a beverage license may not employ anyone under 18 (Fla. Stat. 562.13(1), read October 10, 2026). The restaurant exception covers only workers who are 17 or older, or high-school seniors or graduates with the principal's written permission, and none of them may sell, prepare or serve the alcohol (562.13(2)(d)). If anyone under 18 works there, stop scheduling them until each one fits the exception, with the paperwork on file. Question for you: Does anyone under 18 work at Frozen Fusion? If yes, what are their ages and duties?
3. Jacksonville business tax receipt for 2026-27. It was due September 30, 2026. From October 1 a 10% penalty applies, plus 5% a month, capped at 25% (Fla. Stat. 205.053(1), read October 10, 2026). If it is unpaid, pay it now: the penalty is already running. Question for you: Was the 2026-27 business tax receipt paid by September 30? Please send it.
4. The $500 drawing and the review page. The $500 review drawing has official rules, run by the ordering company Popmenu: no purchase needed, U.S. residents 18 and older, entries close October 31. The rules are one click from your reviews page. Your home-page banner ("Win $500! Enter by writing a dish review to help others. Drawing 11/1.") does not show the main terms or a link to them. Florida law requires every ad for a drawing to show them (Fla. Stat. 849.094(3)). Add this line to the banner: "No purchase necessary. U.S. residents 18+. Enter by Oct 31. Prize: $500 gift card from Popmenu. Rules:" followed by the link to your official rules page, added before you post. Also mark reviews written for the drawing: "Entered in a $500 giveaway for writing this review" (16 CFR 255.5). Question for you: Did Frozen Fusion sign up for the Popmenu review giveaway, and who approved the banner?
5. The July 29, 2026 inspection complaint. The state record shows a complaint visit on July 29, 2026: follow-up required, milk held at 48 to 53°F, a stop-sale, marked as a repeat with an administrative complaint. The July 30 visit met standards. Keep every log, invoice and message from that week until the complaint is closed, and send us the outcome. Question for you: How was the July 29 administrative complaint resolved (any fine or settlement), and what proof of each correction do you have?
6. Workers' compensation insurance. The state proof-of-coverage lookup (data as of October 10, 2026, read October 11, 17:12 UTC) shows "No Policy Currently In Effect" for FROZEN FUSION LLC. It also shows one officer exemption of type Construction (February 24, 2026 to February 24, 2028), which is unusual for a café. Florida requires a policy once a non-construction business has 4 or more employees (Fla. Stat. 440.02(20)(b)2.). If 4 or more people work there, counting part-time, a policy is required now. Ask your insurance agent to confirm the exemption type fits. Question for you: How many people work at Frozen Fusion, counting part-time, and does the business do any construction work?
What the owner may not know
- Two of your five state inspections needed a follow-up. April 20, 2026: 2 high-priority, 4 intermediate and 2 basic violations. July 29, 2026: a complaint visit with 3 high-priority and 4 basic violations, milk held at 48 to 53°F and a stop-sale, marked as a repeat. The visits on May 28 and July 30, 2026 met standards (state licensing board record, read October 10, 2026).
- Staff training proof was missing once. On April 20, 2026 the inspector wrote: "No proof of required state approved employee training provided for any employees." Florida requires that proof to be shown on request, at every inspection (Fla. Stat. 509.049(5)).
- Your two licenses don't show the same address. The food service license reads "Ste 19", and the beer-and-wine license reads "10503 San Jose Blvd" with no suite. The company website (read Oct 11, 2026) shows Suite 19A. Ask the owner which is right. Delivery drivers, customers and inspectors may be sent to different doors.
- Email sent from your web address would likely land in spam. frozenfusionus.com has no mail server and no SPF sender record, while its DMARC policy tells receivers to quarantine mail that fails (read October 10, 2026; test search: the same lookups find the mail and sender records of mozilla.org).
- Florida's minimum wage became $15.00 an hour on September 30, 2026 (Fla. Const. art. X, s. 24(c)).
- The public money records are clean. No secured-loan filing (UCC), no lien or judgment in Duval County records (2010 to October 7, 2026) and 0 federal court cases under "Frozen Fusion" (test search: the same three searches find Publix Super Markets). State court cases were not covered.
The first 10 moves (rebuild plan)
- This week: have GoDaddy lift the renewal lock, renew frozenfusionus.com for several years and turn on auto-renew.
- Today: answer the under-18 question, and stop scheduling anyone under 18 who does not fit the exception.
- Pay or send the 2026-27 Jacksonville business tax receipt.
- Send the $500 drawing's rules and entry method; drop any review or purchase step from future giveaways.
- Send the outcome of the July 29 complaint and the proof of each fix.
- Start one training binder: each manager's food-protection certificate (due within 30 days of hire), each worker's training (within 60 days) and the signed illness-reporting forms.
- Keep a written temperature log for each cooler at opening, midday and close.
- Confirm the real suite, ask the beverage division to add "Suite 19", and use one name, address, phone and hours card everywhere.
- Fix email on the web address: publish an SPF record (or set up company email), so mail from your name is delivered.
- Answer the owner questions at the end of this report so the areas with no letter can be graded.

Fix in the next 30 days
3. CAA record (which certificate companies may issue for the domain). The DNS administrator adds a CAA record naming the company that issues the site certificate (ask the website vendor which one). 15 minutes (estimate). NEXT 30 DAYS.
4. Email authentication (SPF, DKIM, DMARC). Owner confirms whether anyone sends email from an @frozenfusionus.com address. If nobody does: the DNS administrator publishes `v=spf1 -all` (5 minutes (estimate)) and later moves DMARC to reject. If someone does: add that sender's SPF and DKIM first. NEXT 30 DAYS.
C10 Investor offerings (SEC). Has anyone invested money in the business, or lent it money outside a bank? Owner: business owner. Cost: $0 (estimate).
3. Licenses and inspections. 1. Owner sends the outcome of the July 29 administrative complaint (any fine or settlement) and proof of each correction. 2. Managers must pass an approved food-protection test within 30 days of hire, with proof shown at inspection (Fla. Stat. 509.039, read October 10). Staff must be trained within 60 days of hire; training lasts 3 years (509.049, read October 10). Owner sends both lists. 3. Ask the beverage division to add "Suite 19" so both licenses match. NEXT 30 DAYS.
13. Money owed and public records. Next route for state court cases: a browser search of the Duval Clerk court records. NEXT 30 DAYS.
T1 Staff food-safety training records kept and shown on request. one training binder or folder, with a copy at the front counter: each worker's certificate, hire date and training date, checked against the 60-day deadline every month
T3 Employee illness-reporting acknowledgements on file. each worker signs the illness-reporting form at hire; the signed forms are kept in the training binder

Here: 12 company pages = 12. Not counted: Bing Places, Nextdoor, LinkedIn (no listing came up) and YouTube (no channel found and your website does not link one).

Search trail: DuckDuckGo (22:08:42 UTC and 22:12:44 UTC) returns the Facebook, Instagram and X pages and posts from them; a Nextdoor business page exists at nextdoor.com/pages/frozen-fusion/ (DuckDuckGo 22:13:11 UTC; Nextdoor is closed to us by team rule, so only the search result was read). A post in the public Facebook group "The Scoop with Jax Restaurant Reviews" promotes the shop (search result only). Comments, tagged posts and the Facebook reviews tab sit behind a login, and we do not log in to a client's accounts, so no comment was read. Ask the owner: "In the last 12 months, has anyone posted a complaint, a bad review or a negative comment on your Facebook page (including Reviews), Instagram, TikTok, X or Nextdoor? Please send a screenshot of each one and of your reply, and tell us the date of your last post on each account." | Owner: shop owner sends the screenshots; we grade each comment and reply on receipt. | R7 News and press | D | FOUND, negative: FloridaFoodSafety.org, "Food from unknown sources, an unreported sick worker — and Jacksonville's Frozen Fusion stayed open anyway" (page title "Jacksonville Frozen Fusion Racked Up 6 High-Severity Violations, Stayed Open"), August 12, 2026, "FloridaFoodSafety.org Staff"; the page says it was written with AI from state inspection data and reviewed by an editor (floridafoodsafety.org/news/inspections/frozen-fusion-health-inspection-august-2026, read October 11). It does not quote the shop. It does not match the state record. The article says July 29 brought six high-severity violations and one intermediate, naming food from an unapproved or unknown source, an employee not reporting illness, poor handwashing, unsanitized food-contact surfaces, no raw-food consumer advisory, an absent person in charge and reused single-use items. The state's own record for that visit lists 3 high-priority, 0 intermediate and 4 basic: raw animal food not separated from ready-to-eat food; a stop-sale on milk, plant milks and whipped cream held overnight at 48 to 53°F; ice-cream mix out at 46°F (marked repeat violation and administrative complaint); plus mold in the ice machine, dead insects in traps, freezer ice buildup and a knife stored in a crack (DBPR visit detail, saved October 8; re-read October 11, 22:17 UTC). None of the article's sourcing, illness, handwashing, surface, advisory or person-in-charge items appears on the state record. This article is the first result for the shop's name with "complaint", "news" and "reddit" on DuckDuckGo (22:08:42 to 22:13:02 UTC). FOUND, neutral: NewsBreak "Frozen Fusion Offers Brazilian Desserts and Coffee" (22:12:44 UTC); the Times-Union's inspection data page data.jacksonville.com lists the shop's inspections (search result 22:08:48 UTC; the page blocked automated reading). NOT FOUND on local TV and newspaper sites: a web search for the six local outlets (October 11), 0 mentions; control: the same searches return the floridafoodsafety article and event listings for the shop. | Owner: shop owner, with the shop's attorney if wanted. 1. Write to the site's editor (named on the page), attach the state's July 29 record, and ask for a correction: the counts (3 high, 0 intermediate, 4 basic) and the removal of the violations the state did not cite; ask that it add that the July 30 re-inspection met standards. 2. Keep inspections clean. Cost: $0, 30 minutes (estimate); attorney letter about $150 to $400 (estimate). Priority: NEXT 30 DAYS. |
| R8 State complaints and discipline | F (0.0) | Read October 8 and October 10. Five inspections on record for food license SEA2616528. **July 29, 2026 (complaint visit):** follow-up required; 3 high-priority, 0 intermediate, 4 basic violations; milk held at 48 to 53°F, raw animal food stored with ready-to-eat food; a stop-sale; marked as a repeat with an administrative complaint noted. **July 30, 2026:** met standards, no violations. **April 20, 2026:** follow-up required (2 high, 4 intermediate, 2 basic); **May 28, 2026:** met standards. **October 29, 2025:** met standards (0 high, 1 intermediate, 2 basic). Test search: the same record route shows the seven July 29 violations, so the clean visits are real results. A third-party site lists the same five visits and 18 violations in all, which matches our counts (7, 0, 8, 0, 3). The five inspection records do not show what happened to the administrative complaint (fine, settlement or dismissal). Ask the owner: "How was the July 29, 2026 administrative complaint closed (fine paid, settlement or dismissed), and can you send the final order or letter from the state?" Severity 3 (regulator action). The record shows clean re-inspections on July 30 and May 28, but the administrative complaint outcome is not on file, so under the rubric (line 37) a regulator action is F until the outcome is shown closed. | Owner: shop owner, with the shop's attorney. 1. Sends the outcome of the July 29 complaint and proof of each correction (temperature logs, food-separation setup, training). 2. Keeps the clean-inspection run going: zero high-priority violations in each of the next 3 inspections, plus a closed complaint with evidence, lets us re-grade this row to D or better. Cost: $0 to send; attorney review about $150 to $400 (estimate). Priority: RISK NOW if the complaint ended in a fine not yet paid; otherwise NEXT 30 DAYS. |
| R9 Court records | Ask the owner | FOUND 0 so far: federal court records 0 cases for "Frozen Fusion"; Florida UCC index no "Frozen Fusion" debtor; Duval Official Records, every document type, 2010 to October 7, 2026: "No names found" (October 10, attested by our team; controls: the same searches find Publix Super Markets, 2,584 federal docket entries, UCC filings, 609 Duval record names). State court (civil, small claims, employment): the Duval Clerk's CORE PublicAccess search asks for a captcha before any case search (debt-credit row D11.9, read 17:08 UTC; our check 22:10 UTC showed the login page), and we do not solve captchas. Search trail: a web search for "Frozen Fusion LLC" Florida lawsuit OR plaintiff OR defendant (October 11): no case names the company. Ask the owner: "Has Frozen Fusion LLC ever been sued, taken to small claims, or received a court summons or a demand letter? If yes, please send the first page." | Owner: shop owner answers; or a person at a screen runs the Duval CORE PublicAccess name search (captcha by hand). Cost: $0, 15 minutes (estimate). | R11 Review authenticity (FTC 16 CFR 465) | C (2.0) | Home page of www.frozenfusionus.com, read October 11, 2026, 17:00:32 UTC: a banner at the top says "Win $500! Enter by writing a dish review to help others. Drawing 11/1." The review form on /reviews (read 17:00:47 UTC) asks for the location, the item, the review and an email, and says Popmenu, "which powers this site and is sponsoring this giveaway", runs it. The form does not ask for a star rating or a positive review, so it does not break the rule against rewarding only good (or only bad) reviews (16 CFR 465.4, eCFR current to October 7, 2026, read October 11). The gap: the four reviews shown under "Our Latest Reviews" (all positive, 6 to 7 months old) carry no note that reviewers could win a prize. FTC guidance says a review written for an incentive should say so clearly (16 CFR 255.5(a) and its Example 6(ii), read October 11). The site shows no star average and no low reviews; whether low reviews are held back is a separate rule (16 CFR 465.7(b)). Ask the owner: "Does Popmenu publish every review sent through your site, including low ones, or do you or Popmenu choose which ones show? And did any entrant have to give a good review to win?" | Owner: shop owner, with Popmenu. 1. Ask Popmenu to add a line to each review from the drawing, such as "Entered in a prize drawing". 2. Show low reviews as well as good ones, or say on the page how reviews are picked. 3. Keep the drawing open to every review, good or bad. Cost: $0; attorney review of the Popmenu giveaway terms about $150 to $400 (estimate). Priority: NEXT 30 DAYS; RISK NOW only if the owner's answer shows entries were tied to a good review. |
| R12 Other complaint sites (Trustpilot, Ripoff Report, Complaints Board) | B | NOT FOUND (B-capped). Trustpilot, Ripoff Report, Complaints Board and Pissed Consumer: a web search limited to those sites for "Frozen Fusion" review complaint, and for frozenfusionus.com, found 0 pages about the shop (October 11, 22:19 UTC). Control: the same limited search for Jacksonville company complaints found 10 pages (8 Ripoff Report, 2 Complaints Board), so the route finds complaints on those sites. Ripoff Report: DuckDuckGo "frozen fusion jacksonville ripoff report" (22:13:02 UTC) shows no report, only the site home page. Complaints Board and Pissed Consumer: DuckDuckGo "Frozen Fusion" Jacksonville complaint (22:08:48 UTC), 9 results, none on a complaint site. Control: the same DuckDuckGo route returns the shop's BBB profile and inspection pages, so it finds pages about this name. Scope: first page of each search. | Owner: shop owner. Add these sites to the name alert in R6. Cost: $0, 10 minutes (estimate). Priority: NEXT 90 DAYS. |
| R14 Search-suggestion test | B | FOUND, read October 11, 2026, 22:04:14 to 22:04:18 UTC: Google, Bing and DuckDuckGo suggestions for "frozen fusion" plus jacksonville, reviews, complaints, scam, lawsuit, cafe, health and closed show no negative phrase. Google offers "frozen fusion jacksonville reviews", "frozen fusion jax", "frozen fusion menu"; for "complaints" and "scam" it only offers "frozen fusion reviews"; for "lawsuit", "health" and "closed" it offers nothing. Control: the same route returned 10 suggestions for "frozen fusion", so it ran. Two notes: (1) "frozen fusion" is also the name of a cannabis strain and other products, which crowd the suggestions without "jacksonville"; (2) on page one of results, the negative FloridaFoodSafety article (R7) ranks first for the name with "complaint" and "news" (DuckDuckGo 22:08:48 and 22:12:44 UTC). Graded in R7, not twice. | Owner: shop owner. Use "Frozen Fusion Jacksonville" or "Frozen Fusion Cafe" in every profile and post, so searches find the shop and not the strain. Cost: $0. | P1 Peer set: 3 named local competitors | ESTIMATE: confirm with owner | Starter peers, all within about two miles on San Jose Boulevard (a search-engine roundup, October 10): V Pizza, Whit's Frozen Custard, Menchie's. Others named: Bruster's Real Ice Cream, Jeremiah's Italian Ice. "Top" is not yet backed by a ranking or the owner's view. | Owner: shop owner. Picks the 3 shops customers compare with. Cost: $0, 15 minutes (estimate). Priority: NEXT 30 DAYS. |
| P2 Competitors' published prices | D | All three peers read on their own websites. V Pizza menu page (October 10, 20:33:47 UTC) prints only a 48-wing platter at $94 and a pizza stone at $20; no pizza, panini or dessert price. Whit's Frozen Custard menu page (October 10, 20:33:52 UTC) prints no dollar amount and says "(Pricing & Product Vary By Location)". Menchie's home page (robots 200 and allowing, read October 11, 2026 17:06:47 UTC) prints no per-ounce or cup price; its only dollar figure is a free $5 birthday reward. Result: 0 of 3 peers publish a price we can compare to a Frozen Fusion item. Control: the same reads return the $94 and $20 figures and the Menchie's reward text, so prices would have been seen. Scope: home plus menu pages, text only (prices inside images or ordering apps are outside the read). | Owner: write down 10 common items at each of the 3 shops from their in-store boards, dated; a staff member, about 1 hour (estimate), $0. NEXT 30 DAYS. |
| P3 Frozen Fusion's own published prices | B- | Prices print on the ordering page (Oct 11, 17:12 UTC), e.g. Four Cheese Pizza $19.99, Dulce de Leche crepe $12.99, cakes $6.75. The earlier note that menus were unavailable is out of date. | None needed. |
| P7 Fee disclosure and wording (delivery, service, platform fees) | C | The ordering page reads, but fees show only at checkout; a test order needs separate approval. | The website provider sends a screenshot of checkout showing every fee, or you approve one test order. |

The banner is "Win $500! Enter by writing a dish review to help others. Drawing 11/1.", a promotion and not a sales claim. One unprovable superlative keeps this at B-. If yes, please send the first page." | Owner: shop owner answers; or a person at a screen runs the Duval CORE PublicAccess name search (captcha by hand). Cost: $0, 15 minutes (estimate). Priority: NEXT 30 DAYS. |
Cost: $0. Priority: NEXT 90 DAYS. |

| F14.9 Public unpaid-tax warrants, liens and judgments against the company | ESTIMATE (A-; confirm with owner; left out of the mean) | Department of Revenue delinquent taxpayer list, read October 10: no entry for "FROZEN" or "FUSION" in about 1,990 rows. Test search: the same search on the same file finds "PORTABELLA RESTAURANT INC". Florida secured-transaction index: no "FROZEN FUSION" debtor (the list runs from "FROZEN FUN, LLC" straight to "FROZEN GOLD LLC"). Duval County Official Records, 2010 to October 7, 2026, every document type: "No names found". Test search: the same record search finds Publix Super Markets. The state list shows only warrants of $100,000 or more, so a smaller unpaid balance would not appear. Not covered: state court cases and federal tax liens filed in other offices. | Owner answers: has the IRS or the State of Florida ever sent a lien, levy, warrant or unpaid-tax notice? Send any letter. 10 min (estimate). NEXT 30 DAYS. |
| F18.3 Bankruptcy filings | Not found in open sources | CourtListener bankruptcy search (Oct 11, 22:03-22:07 UTC): "Frozen Fusion" 0 results, "Frozen Fusion LLC" 0 results; the same search for Publix returned 2,584. Two web searches returned only company directories. The federal index itself needs a paid login. This does not prove a clean record. | Ask the owner: has Frozen Fusion LLC, or its owner for a business debt, ever filed for bankruptcy or been named in a bankruptcy case? |
| F20 Unclaimed property owed to the company | Ask the owner: will you search "Frozen Fusion" on claimyourcashfl.gov (Florida's free unclaimed-property search) and send a screenshot of the result, or tell us if you have already claimed money from the state? | This search is made by the business itself on the official site (claimyourcashfl.gov); it takes about 2 minutes. Public dig, October 11, 2026, 22:07:01-22:07:12 UTC: DuckDuckGo and Bing searches for "Frozen Fusion" unclaimed property Florida Jacksonville returned no unclaimed-property listing for this company (only a Tripadvisor restaurant page and generic state pages); the official search at portal.claimyourcashfl.gov/ClaimSearch is behind a reCAPTCHA that only a person can pass, and Florida publishes no open name list we could reach. NOT FOUND in open sources; owner-only on the official one. Same finding covers checklist row C17. | Owner steps: (1) open claimyourcashfl.gov and choose Search; (2) type Frozen Fusion in the business name box and tick the reCAPTCHA box; (3) press Search; (4) screenshot the result list, or write "no results"; (5) send the screenshot. Do the same for "Frozen Fusion LLC". 10 min (estimate). NEXT 30 DAYS. |

| B25 Name collision | Checked | A search of the name plus Jacksonville returns only Frozen Fusion at 10503 San Jose Blvd; other "fusion" names found are in other cities (two searches, Oct 11). | None needed. |
| B28 What a stranger and an AI answer say | Checked | A search of the business name returns a BBB listing, a Florida food-safety inspections page and event pages; the company site and review pages were not in the top 10. | Get the site and Google profile ranking for the name search (see Search rows). |

| 1. DNS hosting | A | Name servers are 4 Amazon Route 53 servers on 4 different suffixes (.org,.co.uk,.com,.net), same answer on both resolvers. Test search: the same query path returned the DMARC record and 2 address records, so the lookups worked. | None. |
| 2. DNSSEC (signed DNS answers) | C | No DS record at the parent and no DNSKEY record, on both resolvers, so the domain is unsigned. Test search: cloudflare.com returned a DS record through 1.1.1.1 at. Unsigned is common for small businesses; it leaves DNS answers open to forgery on the network path. | The DNS administrator turns on DNSSEC signing in Route 53 and adds the DS record at GoDaddy. Under 1 hour (estimate). NEXT STAGE. |
| 3. CAA record (which certificate companies may issue for the domain) | C | No CAA record on either resolver. Test search: google.com returned 0 issue "pki.goog" at. Without it, any public certificate company may issue a certificate for the name. | The DNS administrator adds a CAA record naming the company that issues the site certificate (ask the website vendor which one). 15 minutes (estimate). NEXT 30 DAYS. |
| 4. Email authentication (SPF, DKIM, DMARC) | B | "_dmarc" returned "v=DMARC1; p=quarantine; adkim=r; aspf=r; rua=mailto:@onsecureserver.net;" on both resolvers, a policy past monitoring. The apex has no MX record and no SPF record (NOERROR with no answer on both; the apex address query answered, so the names exist). DKIM: nine common selector names returned no record (Oct 11, 22:04 UTC, with a control that returned a record for Gmail); that cannot prove DKIM is absent. Ask the owner to send the header of a test email. Test search: mozilla.org returned a "p=reject" DMARC record and an SPF record at. Because there is no SPF, mail sent from this domain would fail DMARC and be quarantined. | Owner confirms whether anyone sends email from an @frozenfusionus.com address. If nobody does: the DNS administrator publishes "v=spf1 -all" (5 minutes (estimate)) and later moves DMARC to reject. If someone does: add that sender's SPF and DKIM first. NEXT 30 DAYS. |
| 5. Website HTTPS, security headers and exposure | Checked in part | HTTPS is forced (http:// answers 301 to https://). Home page headers (read Oct 11, 17:12 UTC): present nosniff, X-Frame-Options SAMEORIGIN, Referrer-Policy strict-origin-when-cross-origin; missing HSTS, Content-Security-Policy, Permissions-Policy. Certificate: an Oct 8 research report shows Google Trust Services WE1, valid to Nov 25, 2026. Public scanners were blocked by the site's bot check, so there is no scanner grade. | Provider adds HSTS, a Content-Security-Policy and a Permissions-Policy. NEXT 30 DAYS. |

| D11.1 D-U-N-S number exists | Not found | The D&B company search blocked our request and a web search returned no D&B profile (Oct 11, 22:03 UTC); only BBB and Florida food-safety listings came up. | Ask the owner: do you have a D-U-N-S number? Or a person runs the free lookup in a normal browser. |
| D11.2 D&B file accuracy (name, address, phone) | Not found | Same search trail as D11.1. Name and address seen elsewhere: the Florida food-safety site lists the shop, latest inspection July 30, 2026. | Ask the owner: send a screenshot of the D&B profile if one exists. |
| D11.5 Experian business file | Not found | Experian's business page blocked our request on October 2, 2026 (not bypassed). A web search on October 12, 00:06 UTC found no Frozen Fusion result. No free public view was found. | Ask the owner: send the Experian business score and date if you have looked. NEXT STAGE. |
| D11.6 Equifax business file | Not found | The Equifax page we tried returned a page-not-found error on October 2, 2026. A web search on October 12, 00:06 UTC found no Frozen Fusion result. | D11.7 UCC filings against the business | A | Florida UCC index (exact-name list), searched October 10, 2026: the 20-row window runs from FROZEN FUN, LLC to FROZEN YUMMY ICE, LLC and holds no FROZEN FUSION debtor (a FROZEN FUSION name would sort between FROZEN FUN and FROZEN GOLD). Test search: the same Florida UCC search for Publix Super Markets returned 12 Publix debtor names in a 20-row window; the same Duval Official Records name search for Publix listed 609 names; the same federal docket search for Publix returned 2,584 entries. Scope: the exact-name list for the name as typed; a debtor filed under a different legal name is not covered. | None. Owner still lists any equipment, vehicle or inventory loan so the lender's filing can be matched later. NEXT STAGE. |
| D11.8a Federal tax liens | A | Florida federal-lien debtor-name list (Secretary of State, dos.sunbiz.org, read October 11, 22:04 UTC), name FROZEN FUSION: the list opens at FROZEN FUZIONS, INC. (a different name and address, not attributed to this business), then FROZEN GUY entries; no FROZEN FUSION entry. Test search through the same form: PUBLIX SUPER MARKETS returned Publix entries. The Duval Official Records search found no lien either (row D11.9r). Scope: Florida filing office, name as typed; liens under another spelling or filed elsewhere are not covered. | Ask the owner: did you ever get an IRS lien or levy notice? NEXT 30 DAYS. |
| D11.8b State tax warrants | Not found, control passed | Florida Department of Revenue delinquent-taxpayer list (read Oct 11, 22:04 UTC, includes Duval): no FROZEN or FUSION entry. The list covers only unsatisfied totals of $100,000 or more plus the top two per county, so a smaller warrant would not appear. | Ask the owner: have you received any Florida Department of Revenue warrant, notice or payment-plan letter? |
| D11.9 Civil suits and judgments in state court (Duval Clerk) | Ask the owner | The Duval Clerk's court-records page (read October 11, 2026, 17:08 UTC) says the public can view non-confidential Duval court records in the Clerk's CORE portal without an account, at the CORE "PublicAccess" entry. That entry asks for a captcha before a case search, and we do not solve captchas, so no state-court search was run. | Ask the owner: Has Frozen Fusion LLC ever been sued, or received a court summons or a judgment, in any Duval County court? We can look it up together with you at the CORE public-access page, where you solve the captcha. 10 minutes. NEXT 30 DAYS. |
| D11.9r Recorded liens, judgments and lis pendens (Duval Official Records) | A | Duval County Official Records, every document type, 2010 to October 7, 2026, name search October 10, 2026: "No names found." Test search: the same Florida UCC search for Publix Super Markets returned 12 Publix debtor names in a 20-row window; the same Duval Official Records name search for Publix listed 609 names; the same federal docket search for Publix returned 2,584 entries. Scope: Duval County recorded documents only (liens, judgments, lis pendens, notices recorded in the county); other counties and state court cases are not covered. | None. NEXT STAGE. |
| D11.10 Federal court cases against the business | A | CourtListener RECAP (federal dockets), "FROZEN FUSION", October 10, 2026: count 0. Test search: the same Florida UCC search for Publix Super Markets returned 12 Publix debtor names in a 20-row window; the same Duval Official Records name search for Publix listed 609 names; the same federal docket search for Publix returned 2,584 entries. Scope: federal dockets that someone uploaded to RECAP; state courts and unuploaded federal filings are not covered. | None. Re-run once a year. NEXT STAGE. |
| D11.11 Bankruptcy filings by the business | ESTIMATE: confirm with owner | CourtListener RECAP search count 0 above, but RECAP holds only filings that someone uploaded, so it cannot show that none exists. ESTIMATE: confirm with owner. If graded it would be a B- (see D11.19 for the same rule). | Owner says whether the business ever filed. 2 minutes (estimate). NEXT 30 DAYS. |
| D11.12 Business identity (entity active, address steady) | A | Florida Division of Corporations quarterly file cordata.zip (generated October 7, 2026), read October 12, 2026, 00:01 UTC: L25000105182, FROZEN FUSION LLC, status A (active), filed March 3, 2025, principal and mailing address 10503 San Jose Blvd, Suite 19, Jacksonville FL 32257, annual report filed February 16, 2026, registered agent LM Accounting & Payroll Services LLC. Suite wording: the Sunbiz record and the state food-service license (D11.18) both read Suite 19; the company website and the BBB listing read 19A. The official filings agree with each other, so Suite 19 is the filing address and 19A is a variant used on the website and BBB. Test search: the same file returned a different entity, DELIGHT FROZEN FUSION LLC (Miami), so the name filter works. Scope: state record as of October 7, 2026; changes after that date are not in it. | Owner picks one suite number (19 or 19A) and makes the website and BBB match the Sunbiz filing. 15 minutes (estimate). NEXT 30 DAYS. |
| D11.16b Other federal loans (EIDL, SBA, Treasury-referred debt) | Not found, control passed | SBA Paycheck Protection Program public data (data.sba.gov, all 13 files, read October 12, 00:04 UTC to 00:15 UTC), business name FROZEN FUSION: about 11.47 million rows read, none contain the name. Test search through the same files: each file returned Jacksonville rows, so the search worked. The company was formed March 3, 2025 (D11.12), after PPP ended (May 31, 2021) and after SBA stopped taking COVID EIDL applications (last day December 31, 2021; portal closed May 16, 2022; sba.gov, read October 12), so no PPP or COVID EIDL loan can exist for this company. Not covered: other SBA loans, Treasury-referred debt (not public by name), or an earlier company. | Ask the owner: has the business ever had an SBA loan or any debt referred to the Treasury? NEXT 30 DAYS. |
| D11.18 Business address type (mailbox, virtual office, working office) | B+ | The premises at 10503 San Jose Blvd, Jacksonville, FL 32257 is a working, state-licensed and state-inspected food-service location, not a mailbox. Florida DBPR license search (data current as of October 10, 2026, 4:17 PM ET): Permanent Food Service license SEA2616528, "Current, Active", expires June 1, 2027, license location, main address and mailing address all "10503 SAN JOSE BLVD STE 19". DBPR inspection list for the same license (read October 10, 2026): five visits, April 20, May 28, July 29 and July 30, 2026 among them. The company website lists the same street with "suite 19A" (home page, read October 11, 2026, 16:58 UTC). Test search: the same DBPR search for Publix Super Markets returned 3,206 records on October 10, 2026, so the route works. Scope: the operating premises only; the address on the company's Sunbiz filing is covered in D11.12. The suite reads 19 on the state license and 19A on the website, which costs this row one step (B+ = 3.3). | Owner picks one suite number (19 or 19A) and makes the state license, website, Google profile and Sunbiz filing say the same. 15 minutes (estimate). NEXT 30 DAYS. |

| 1. Business structure and standing | A | Florida Division of Corporations official quarterly business file (generated October 7, 2026, read October 12, 00:03 UTC): document L25000105182, FROZEN FUSION LLC, Florida limited liability company, status Active, filed March 3, 2025. Principal and mailing address 10503 San Jose Blvd, Suite 19, Jacksonville FL 32257. 2026 annual report filed February 16, 2026. Registered agent: LM Accounting & Payroll Services LLC. The licensing board lists the same company name (re-read October 11, 22:06 UTC). Both state records say Suite 19; the website says Suite 19A (see row 3). | File the annual report each year between January 1 and May 1 (Fla. Stat. 605.0212(3), 2026 text, read October 12, 00:06 UTC); the state charges $138.75 to file, and a $400 late fee after May 1 (Division of Corporations annual report page, read October 12, 2026, 13:02 UTC). A company that does not file cannot bring or defend a lawsuit in a Florida court until it files, and can be dissolved (605.0212(6)). Use one suite number everywhere. |
| 3. Licenses and inspections | B- | Food service license SEA2616528 (seating): Current, Active, expires June 1, 2027. Retail beverage license BEV2612905, class 2COP (beer and wine for drinking on site): Current, Active, expires September 30, 2027. Both read October 10. Inspections (same record, read October 10; five visits, the latest July 30, 2026): April 20, 2026 follow-up required (2 high-priority, 4 intermediate, 2 basic); May 28, 2026 met standards; July 29, 2026 complaint visit, follow-up required (3 high-priority, 0 intermediate, 4 basic; milk held at 48-53°F; a stop-sale; marked repeat, administrative complaint); July 30, 2026 met standards. Test search: the same record route shows the July 29 violations, so the clean visits are real results. The outcome of the July 29 administrative complaint is not public online (the case detail page did not open); Ask the owner for it. The beverage license location reads "10503 San Jose Blvd" with no suite, while the food license reads "Ste 19". |
| 9. Privacy and data security | B- | The footer words "Frozen Fusion Cafe Privacy Policy" are plain text, not a link; only the ordering company's policy opens (last updated March 25, 2022). Florida requires disposal of customer records (Fla. Stat. 501.171(8)) and written consent for automated marketing texts (501.059). | Publish your own privacy policy and link it in the footer; owner confirms how online-order and catering records are disposed of. NEXT 30 DAYS. |
| 10. Advertising and marketing | C | The $500 review drawing has official rules from Popmenu (no purchase needed, entries close Oct 31), one click from your reviews page; the home banner lacks the main terms (Fla. Stat. 849.094(3)). Reviews written for the drawing carry no incentive note (16 CFR 255.5(a)). Paying for only positive reviews is banned (16 CFR 465.4). | Fix risk #4 above. Owner sends the winner record. NEXT 30 DAYS. |
| 12. Name clearance and trademarks | B+ | Federal trademark register, read October 10: 7 records use "FROZEN FUSION" (including FROZEN FUSION FRUIT SMOOTHIES). All 7 are dead (cancelled or abandoned), and none are owned by this company. Test search: the same search returns 194 GOOGLE records, so it works. The Florida trademark register was not searched. | No live federal conflict found. If the brand expands, ask a trademark attorney about registering it. NEXT STAGE. |
| 13. Money owed and public records | A | Florida UCC index, read October 10: no "FROZEN FUSION" debtor (the alphabetical list runs straight from FROZEN FUN, LLC to FROZEN GOLD LLC). Duval Official Records, every document type, 2010 to October 7, 2026: "No names found." Federal court records: 0 cases. Test searches: the same three searches find Publix Super Markets (UCC filings, 609 Duval record names, 2,584 federal docket entries). Not covered: state court cases and unclaimed property. | Next route for state court cases: a browser search of the Duval Clerk court records. NEXT 30 DAYS. |
| 15. Sanctions and exclusions | A | U.S. Treasury sanctions lists (SDN and consolidated), downloaded October 10: no "FROZEN FUSION". Test search: a name known to be listed is found in each file. Federal exclusion list (SAM.gov): the search page bars automated search and the data feed needs a key; a web search found no hit (Oct 11, 22:10 UTC). Ask the owner: have you or any owner ever been barred from federal contracts? | gov exclusion search in a browser. NEXT STAGE. |

| M4 Social media (organic) | Ask the owner | Social page counts were not used. What stands: the company website links Facebook, Instagram and TikTok (frozenfusionjax); web searches returned no profile pages; no YouTube channel is linked. Send us your page insights for each profile so we can grade it. | Owner: tell us who posts and how often, and delete or fill the empty YouTube channel. 20 min (estimate), no cost. NEXT 30 DAYS. |
| M6 Paid ads (Google, Meta) | C | Page code of the home page read October 11, 2026: one Google Ads tag (AW-17925927037) and Google Tag Manager are present, so a Google Ads account exists or existed. No Meta Pixel (0 hits for fbq and fbevents) and no TikTok pixel (0 hits for ttq), so Facebook, Instagram and TikTok ads cannot be tracked to orders. A tag proves the account, not that ads run today. Control: the same file does contain the Google tags, so the scan read the code. | Owner: screenshot the Google Ads account (spend, last 30 days) or say it is off; install a Meta Pixel if Meta ads are wanted. 30 min (estimate), no cost. | P2 Tap-to-call on the website | Checked | A tel link sits in the footer of every page, not above the fold; it is 17-20 px tall at phone width. | Put a call button at the top of the home page, at least 24 px tall. |
| P3 Hours published and the same everywhere | Checked, one conflict | Website: Monday-Sunday 9:00 am - 9:00 pm. | Ask the owner: which hours are right, and do Google and Yelp show the same? |

| Q2 Website works: links, ordering, tap-to-call, phone layout, speed | Run scheduled | Page speed and link check run after 07:00 UTC on Oct 12 (Google's PageSpeed quota resets); real-user speed data is the second route. Ordering, links and tap-to-call were read on the saved pages: ordering works for pickup and delivery; tap-to-call is in the footer (17-20 px tall). | Results land in this row by Oct 12, 08:00 UTC. |
| Q3 Website accessibility (WCAG 2.2 AA quick check) | Checked in part | Saved pages (home, catering, contact; Oct 11, 17:03-17:12 UTC): page language set, one h1 each, no skipped heading levels, 0 images missing alt text, skip link present, an accessibility-assistance widget present. Colour contrast, keyboard use and form labels need a scanner run by a person. | Run an axe scan in a normal browser; enlarge small tap targets. |
| Q4 Email protection on the domain (SPF, DKIM, DMARC) | A- | DMARC policy p=quarantine, alignment relaxed, read on both resolvers October 10. Mail that fakes the domain is sent to spam by receivers that follow DMARC. No SPF record and no DKIM key at the 11 common names we tried. The domain also has no mail server (MX), so it neither sends nor receives email. Reports go to an outside address (@onsecureserver.net), not a business inbox. Test: the same reads find mozilla.org's DMARC (p=reject), its mail servers and its Google DKIM key. | Domain admin: add "v=spf1 -all" (says "this domain sends no mail") and move DMARC to p=reject once confirmed unused for mail. Owner: say who reads the DMARC reports. 15 min (estimate). NEXT STAGE. |
| Q6 Domain and website security | C (RISK NOW) | HTTPS is forced: http:// on both addresses answers 301 to https://. The registry record (October 10 20:29 UTC) shows registrar GoDaddy and transfer, delete and update locks ON. **Registration ends 2026-11-04**; created 2025-11-04, so this is the first renewal. The record also shows "client renew prohibited". Auto-renew cannot be seen publicly. Security headers on the home page (read Oct 11, 17:12 UTC): present nosniff, X-Frame-Options, Referrer-Policy; missing HSTS, CSP, Permissions-Policy. Test: the same registry read works for mozilla.org. | Owner: renew today and turn on auto-renew, then send a registrar screenshot (RISK NOW above). Turn on two-step sign-in at the registrar. 15 min (estimate). NOW. |
| Q16 Website connection security (TLS, certificates) | A+ | Valid certificates for both frozenfusionus.com and www (Google Trust Services, valid August 27 to November 25, 2026, chain checks OK). TLS 1.3 offered. TLS 1.0 and 1.1 refused on both addresses; TLS 1.2 answers. Test: the same client connects to a known TLS 1.0 test site, and gets TLS 1.3 from www.mozilla.org. The certificates are 90-day edge certificates that normally renew themselves. The date to watch is the domain's, November 4 (row Q6). | None needed at this stage. HSTS is the next step (row Q16b). |
| Q16b HSTS and browser preload list | Checked | The home page does not send an HSTS header (read Oct 11, 17:12 UTC). Preload list status: not listed (Oct 10). | Provider turns on HSTS. Joining the preload list is the owner's decision and is hard to undo. NEXT STAGE. |
| Q17 Domain hardening (lock, CAA, DNSSEC) | A+ | Registrar transfer, delete and update locks ON (October 10). No CAA record and no DNSSEC; both are later-stage items. Test: google.com CAA and cloudflare.com DNSSEC records are found by the same reads. | NEXT STAGE: CAA naming the certificate issuers; DNSSEC at the registrar. |
| Q19 Security contact on the website | Checked | /.well-known/security.txt returns 404; the privacy policy lists only a Popmenu support address. | Add a security contact file. |
| Q24 Outside code on the website | Checked | Google Tag Manager, GA4, Google Ads, reCAPTCHA, a Mapbox map and Popmenu; no Meta pixel or chat tool found. | Keep the list; drop the old Universal Analytics ID. |
Records and systems: C (2.00), provisional, 5 of 17 rows graded; 11 info requested, 1 estimate left out
- Staff training proof was missing once. On the April 20, 2026 inspection the state wrote: "No proof of required state approved employee training provided for any employees." The same visit found "No proof provided that food employees are informed of their responsibility to report... information about their health". Neither item shows up again in the four other visits we read.
- Temperature problems came back. October 29, 2025: "No probe thermometer provided to measure temperature of food products." April 20, 2026: butter at 70°F and empanadas at 110°F. July 29, 2026: milk and whipped cream held overnight at 48-53°F, a stop-sale, marked "Repeat Violation" and "Admin Complaint". The visit on July 30, 2026 met standards. Daily temperature logs are the record that shows the fix is holding (common practice, not a state rule).
- The domain is held at GoDaddy and its DNS runs on Amazon's DNS service. The website address points to Popmenu, so the website and ordering page run on Popmenu (public DNS lookup, Oct 10, 2026; the site's own footer says "Powered by Popmenu", read Oct 11, 17:08 UTC). That is three outside accounts. Each one needs a known company login.
- The web address lists no mail server and has no SPF sender record, so email sent to an @frozenfusionus.com address is unlikely to arrive. Test search: the same lookup on a large public business finds its mail server. It does have a DMARC record (p=quarantine). The business email is probably on some other service, and the owner should confirm which one.

| T1 Staff food-safety training records kept and shown on request | C | state inspection record, April 20, 2026 (saved October 8, re-read October 10): "No proof of required state approved employee training provided for any employees." No training item on the October 29, 2025, May 28, July 29 or July 30, 2026 visits (test search: the same search of the April 20 record finds the training item). The law says proof for each worker must show "the name of the trained employee, the date of birth of the trained employee, the date the training occurred, and the approved food safety training program used" and must be shown "upon request, including... at the time of any division inspection" (Fla. Stat. 509.049(5), 2026, read October 10) | one training binder or folder, with a copy at the front counter: each worker's certificate, hire date and training date, checked against the 60-day deadline every month |
| T3 Employee illness-reporting acknowledgements on file | C | state inspection record, April 20, 2026 (saved October 8, re-read October 10): "No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Emailed Form." Not repeated in the later visits we read (test search as T1) | each worker signs the illness-reporting form at hire; the signed forms are kept in the training binder |
| 9.2 Domain will not lapse | D | domain registry record, read October 10, 2026: registered November 4, 2025, expires November 4, 2026 (25 days away), status locks on (delete, renew, transfer, update), including "client renew prohibited"; auto-renew not shown publicly | first ask GoDaddy why renewal is locked (client renew prohibited) and have the lock lifted; then renew for several years, turn on auto-renew and confirm the card on file is current |
| 9.4 Website host and admin login | ESTIMATE | public DNS, October 10, 2026: www.frozenfusionus.com points to ssl.popmenu.com (Popmenu); DNS servers are Amazon's (awsdns); registrar GoDaddy. Who holds each login: confirm with owner | the company owns each account; any provider gets a user seat only Owner question: "Who holds the main login for Popmenu, the Amazon DNS account and GoDaddy: the company, or the website provider?" |
| 9.5 Email on the company domain | C | public DNS, October 10, 2026: no mail server and no SPF sender record for frozenfusionus.com; a DMARC record (p=quarantine) is published (test search: the same lookup on a large public business returns its mail server and its SPF record) | set up email on the company domain (for example orders@ or info@) with the company as admin, so customer and supplier mail stays with the business |
| 9.7 Analytics owned by the company | Ask the owner | Tags exist; who owns the accounts is not public. | Ask the owner: who is admin on each GA4 property? |
| 13.1 Public archive of the website | B | public web archive, read October 10, 2026: 2 page captures in the last 12 months (home page February 7, 2026; ordering page February 19, 2026), none after April 2026 (test search: the same search on a large public business finds a capture in each of the last 13 months) | save a dated copy of the website and menu each time they change |

| S4 Review numbers | Ask the owner | Google: 4.8 stars (read October 12, 2026, 13:05 UTC). Yelp: 4.3 stars from 11 reviews (read October 12, 2026, 13:06 UTC). Both pages were read in a private browser window, not signed in. | Ask the owner: how many Google reviews does your profile show? Please also send a screenshot of your Google reviews sorted lowest-first. We will then list each negative review and the reply it needs. |
| S5 Website content for search | Checked | Home title is "Home - ice cream/coffee shop"; meta description names Jacksonville, lunch, coffee, boba, paninis, Portuguese pizza, milkshakes; canonical is the home address; one JSON-LD block; share image set. | Change the page title to include the business name and the city. |
| S6 Local SEO / map pack | Ask the owner | How your business shows on the Google map comes from your screenshot. | Please show us where you appear for these searches: on your phone, search Google and Google Maps for "frozen yogurt Jacksonville" (and your other main items) and tell us where Frozen Fusion shows, or send a screenshot. Also tell us which name should appear on Google. |
| S8 Technical SEO (crawl, sitemap, structured data, speed) | Checked in part | robots.txt allows all and lists the sitemap. The sitemap has 223 URLs, 204 of them menu items (priority: 207 at 0.7, 2 at 0.8, 1 at 0.9, 13 at 1.0). | Run a full speed and crawl report with a browser-based tool; the site shows a security check to plain requests. |
| S9 Competitor tracking | Ask the owner | Needs the Google results from S1 first. | After S1, list the top 3 by public review count. |
Website turns visitors into calls: No letter yet: 0 of 14 rows graded, not enough to grade; 12 info requested, 2 estimates left out
| What we check | Status | What we saw | Fix plan |
|---|
| WC1 Call to action above the fold | Checked | Phone: an Order Online button and a sticky bar Order Now / Sign In / Sign Up. Desktop: ORDER ONLINE in the menu and hero. No call or catering button above the fold. | Add a call button above the fold. |
| WC2 Click-to-call | Checked | tel link in the footer on every page, not above the fold; 17-20 px tall at phone width. | Move it up and make it at least 24 px tall. |
| WC3 Phone consistency | Ask the owner | One number, (904) 728-4747, on every site page read. Ask the owner: what phone and suite do your DoorDash and Uber Eats pages show, and is Uber Eats still open? | Send a screenshot of each store page so we can match the phone and suite to the website. |
| WC4 Form length | Checked in part | The contact form asks first name, last name, email, phone and comments; the catering form has 9 fields including hidden spam-check fields (Oct 11, 17:03 UTC). Ask the owner: how many fields do you see on the catering form? | Keep the catering form to the fields you need (date, time, servings, email, phone). |
| WC5 Form placement | Checked | Forms on /catering, /contact, /private-events (spam check) and a review form on /reviews; none on the home page. | Add a catering button on the home page. |
| WC6 Chat / text | Checked, none | No chat or text widget in the code of 6 pages at phone and desktop widths. | Optional. |
| WC7 Booking / ordering path | Checked | Online ordering works for pickup and delivery on the Popmenu ordering page, with public prices; catering is a form (Oct 11, 16:58-17:12 UTC). The earlier note that menus were unavailable was a stale saved copy. | None needed. |
| WC8 Trust badges that are true | Checked, none shown | No badges, awards or licence claims on the pages read. One boast: "Jacksonville's most exciting frozen dessert destination". | Ask the owner: do you want licence SEA2616528 or any certificate shown on the site? Send the paper first. |
| WC9 Social proof matches | Ask the owner | Site /reviews shows customer reviews with first names and no star average. Ask the owner: what star number and review count do Google and Yelp show? We will check that the site matches them. | Send a screenshot of each page. |
| WC13 Mobile usability | Checked | Phone width: correct viewport, no sideways scroll, no text under 12 px; 13/8/13 tap targets under 24 px on home/catering/contact. | Enlarge small links. |

Keeping it fixed: monthly care plan, $400 a month
After the fixes above, Sandra Restored It can keep the shop moving forward every month.
We fix what this report finds, then keep it fixed. Setup package: $2,700. Monthly care plan: $400 a month with a minimum term of 6 months ($2,400), which can't be cancelled mid-term. Exact scope is agreed in writing before any work starts.
Listings and hours kept current. Google, Apple Maps, Bing, Yelp and the delivery apps show the same name, suite, phone and hours, checked every month, with an alert when one of them changes.
Review replies. A drafted reply to every new Google and Yelp review for you to approve, and a plan for the hard ones.
Menu and allergen claims. Every claim on the menu, the website and the posts is one you can back up, and the allergen wording matches everywhere.
Health-inspection follow-up. A folder for each inspection report and the proof of each fix, plus reminders before the food service license (June 1, 2027) and the beer-and-wine license (September 30, 2027) come due.
Giveaway rules. Every drawing carries the rules line, "No purchase necessary", the end date and a label on reviews written for it.
A full SEO program. A mapped-out plan to make the shop as visible as we can on Google for searches like "boba Jacksonville" and "breakfast near San Jose Blvd". No one can promise a #1 spot on Google, and Google itself warns against anyone who does.
Daily social media content. Content ideas, written posts and short video scripts for every day. Your team posts them from your own accounts, so no account is put at risk. You approve the monthly plan first, and every post carries the name Frozen Fusion Cafe.
Website updates twice a month and a weekly analytics chart. Menu, hours and event changes on frozenfusionus.com, plus website visits, Google searches and views, calls and direction requests every week. You give us view-only access in writing at sign-up.
Small things that add up. An alert when a law or rule in this report changes, a one-page monthly report and a fresh scorecard every 3 months.

Technical reports: SEO, site map and security
The real reports, opened from this page. Each one has a short list of what to change.
| Report | What it shows | What to change |
| Lighthouse, phone | Speed, search setup, accessibility and best-practice scores on a phone | Scores out of 100, run October 12, 2026 at 14:02 UTC on a phone-size screen: speed 80, accessibility 100, best practices 79, search setup 100. The slow part: the main picture takes 4.5 s to appear (Google's "good" line is 2.5 s) and scripts block the page for 220 ms. To change: ask the website provider to cut the unused script (the report estimates 761 KiB) and to shrink the top picture; best practices loses points for 3 third-party cookies. |
| Lighthouse, computer | The same scores on a computer | Scores out of 100, run October 12, 2026 at 14:03 UTC on a computer-size screen: speed 98, accessibility 100, best practices 78, search setup 100. The page shows its main picture in 1.1 s and nothing blocks it. To change: best practices loses points for third-party cookies and browser warnings; ask the website provider which ones come from the ordering platform and which can be removed. |
| Security scan (Observatory) | Which browser protections the website turns on, and the grade it earns | No grade. The public scanners (Mozilla Observatory, securityheaders.com) were stopped by the site's bot check on October 11, 2026 at 22:05 UTC, and a scanner that reads the check page instead of the site would give a false grade. The site's own answers are in the "Site headers" row below. Ask the website provider to allow the scanner, or to turn on HSTS, a Content-Security-Policy and a Permissions-Policy. |
| Site headers | The raw answers the website gives every browser (home page, read Oct 11, 2026, 17:12 UTC). Present: nosniff, X-Frame-Options SAMEORIGIN, Referrer-Policy strict-origin-when-cross-origin. Missing: HSTS, Content-Security-Policy, Permissions-Policy. | Ask the website provider to turn on HSTS and add a Content-Security-Policy and Permissions-Policy. |
| Site map | The site lists 223 pages: 204 menu items and 19 others (about, catering, contact, digital gift cards, events, follow us, gallery, join our team, landing page, menu, order online, private events, reviews and menus). Priorities: 207 at 0.7, 2 at 0.8, 1 at 0.9, 13 at 1.0 (read Oct 11, 2026, 16:58 UTC). The link-by-link crawl map comes with the speed run. | Remove the "landing-page" URL if it is unused and set real priorities; add titles to the pages in the map. |
